复合保温对腹腔镜异位妊娠手术应激反应的影响(1)

【摘要】 目的 评价复合保温对腹腔镜异位妊娠手术患者应激反应的影响。方法 选择行腹腔镜异位妊娠患者168例，随机分成A, B两组。其中A组为复合保温组(实验组)，B组为常规保温组(对照组)，每组84例。分别记录入室前后鼻咽温度T，两组入室后肾上腺素和去甲肾上腺素含量，及送化验科检测凝血功能(分别检测PT,APTT,FBG)。结果 ①两组患者年龄、性别、体重、气腹压力、气腹时间、输液量、冲洗量、室温等组间比较差异无统计学意义(P>0.05)；②术后A组麻醉复苏时间短。③A组患者肾上腺素气腹后和术毕升高的幅度比B组小，两组比较，差异有统计学意义(P>0.05)。A组患者气腹后的去甲肾上腺素明显低于B组，术毕比较差异无统计学意义(P>0.05)；④两组患者入室的PT, APTT, FBG比较差异无统计学意义(P>0.05)，术毕均延长，A组延长的幅度小，术毕与入室相比差异无统计学意义(P>0.05)。B组延长的幅度大，术毕明显高于入室水平。组间比较，术毕各项指标B组明显高于A组差异有统计学意义(P>0.05)。结论 围术期患者应用复合保温措施有利维持异位妊娠腹腔镜手术中体温的稳定,减轻腹腔镜手术中的应激反应。

【Abstract】 Objective To estimate the effect of compound incubation on the patients’ stress reaction during the ectopic pregnancy laparoscopic operation. Methods 168 patients who had ectopic pregnancy laparoscopic operation were divided into A and B grougs.A group was in compound incubation(experimental group) and B group was in normal incubation(control group),each had 84 patients.We recorded nasopharyngeal temperature of the two groups separately and the contents of AD and NE in the room; we tested PT APTT and FBG in chemical examination department. Results ①The patients in the two groups had non-statistic difference in age, gender, weight, the pressure and time of pneumoperitoneum,the volume of transfusion and douche, room temperature, there were no statistic difference between the two groups;②After operation,the patients in group A had shorter palinesthesia-time and less shakes than that in groep B;③The elevated extents of AD after pneumoperitoneum and operation in group A were less than that in group B, and both of which were higher obviously after pneumope ritoneum than that before operation..Compareing within the NE after pneumoperitoneum two groups of NE after pneumoperitoneum, group A was lessobviously than group B,there were no obvious difference between the two groups; ④The PT,APTT,FBG before operation in the two groups were non-different,which changes of prolong extent after operationin group A were less than that in group B,there were non-obvious difference in group A,but that in group B was different compared with group A,and there were obvious difference between the two groups.Conclusion Using compound incubaion,it can keep the temperature constantly and reduce the patients’ stress reaction during the ectopic pregnancy laparoscopic operation. ......
