[摘要] 目的：观察改良序贯疗法在溃疡活动期根除幽门螺杆菌(Hp)的疗效。方法：将确诊为Hp阳性的90例患者分两组，其中标准三联治疗方案45例，奥美拉唑静脉点滴+克拉霉素+阿莫西林，每日2次口服，共10 d；改良序贯疗法方案45例，前5 d奥美拉唑+左氧氟沙星静脉点滴，后5 d奥美拉唑静脉点滴+阿莫西林+甲硝唑，每日2次口服。疗程结束后4周复查Hp检测。结果：三联疗法组Hp根除率为80.0%(36/45)，序贯疗法组Hp根除率为95.6%(43/45)，两组疗效比较，差异具有统计学意义(P<0.05)。结论：改良序贯疗法在根除溃疡活动期Hp感染疗效上明显优于标准三联疗法，可以达到尽早、尽快、有效地根除Hp的目的。

Improved sequential therapy in active ulcer efficacy of Helicobacter pylori eradication

[Abstract] Objective: To observe the effect of the improved sequential therapy in the treatment of eradicate ulcer with active Helicobacter pylori (Hp). Methods: The diagnosis of Hp-positive 90 patients were divided into two groups, one standard triple therapy in 45 cases, intravenous omeprazole + clarithromycin + amoxicillin, twice daily orally for 10 days; improved sequence consistent treatment program in 45 cases, the first 5 days intravenous omeprazole + levofloxacin, then 5 days later intravenous omeprazole + amoxicillin + metronidazole, twice daily orally. Four weeks after the end of treatment review Hp detection. Results: Hp eradication rate of triple therapy group was 80.0% (36/45), sequential therapy group Hp eradication rate was 95.6% (43/45), the two groups was statistically significant difference in efficacy (P<0.05). Conclusion: The modified sequential therapy in the eradication of Hp infection and active ulcer on the superior efficacy of standard triple therapy, can be achieved as soon as possible, as quickly as possible, the purpose of effective eradication of Hp.

幽门螺杆菌(Helicobactor.Pylori，Hp)感染为消化性溃疡的重要病因，凡有Hp感染的消化性溃疡，无论初发或复发，活动期或静止，有无并发症，均予以根除Hp治疗[1]。随着耐药菌不断增多，标准三联疗法根治率下降，序贯疗法更多被应用。本治疗在随机给予改良，采用静脉及口服用药序贯疗法和标准三联疗法根除Hp比较疗效，探讨溃疡活动期尽早、尽快、有效地根除Hp的疗效及方法。
