[摘要] 目的 探讨单侧腰-硬联合麻醉用于80岁以上患者髋关节手术的效果。 方法 回顾性分析2010年2月～2011年10月来本院进行髋关节手术的患者122例，随机平均分为对照组和观察组，对照组患者采用硬膜外麻醉，观察组患者采用单侧腰-硬联合麻醉，比较两组患者的麻醉效果、HR、SBP、DBP、MAP、心电图、SpO2、麻醉起效时间、阻滞完善时间以及用药量。 结果 观察组患者在麻醉起效时间、阻滞完善时间以及1.5%利多卡因用药量明显少于对照组，差异有统计学意义(P < 0.05)；在收缩压(SBP)、平均动脉压(MAP)、舒张压(DBP)等方面比较，观察组患者明显较对照组稳定，麻醉效果明显好于对照组，两组比较差异具有统计学意义(P < 0.05)。 结论 单侧腰-硬联合麻醉进行髋关节手术，具有用药少、起效快、阻滞完善，患者在手术过程中血压平稳，术后并发症少，减少肺部感染，降低深静脉血栓形成，提高手术效果，值得临床广泛使用。

[Abstract] Objective To investigate the effects of unilateral waist-epidural anesthesia in hip joint operation for the patients over 80 years old. Methods Retropectively analyzed the hip joint operation patients from February 2010 to October 2011 in our hospital, randomly divided them into control group and observation group. In the control group, they were given epidural anesthesia, while in the observation group, they were given unilateral waist-epidural anesthesia. The anesthetic effect, HR, SBP, DBP, MAP, ECG, SpO2, onset time of anesthesia, retardant improvement time and dosage of the two groups were compared. Results The onset time of anesthesia, retardant improvement time and 1.5% lidocaine dosage of the observation group were obviously less than those of control group, there were significant differences between them (P < 0.05). The systolic blood pressure (SBP), mean arterial pressure (MAP), diastolic blood pressure (DBP) of the observation group were obviously more stable than those of the control group, and the anesthesia effect was significantly better than the control group, there were significant differences between them (P < 0.05). Conclusion The unilateral waist-epidural anesthesia in hip joint operation has the advantages of less medication, rapid onset of complete retardant, and the patients' blood stress is stable during the operation, has fewer complication, can reduce deep vein thrombosis and improve surgical results, which is worthy of widespread use on clinical ......
