老年糖尿病患者护理中应用个案管理护理模式的可行性(1)
[摘要]目的 探讨个案管理模式在老年糖尿病患者护理中的可行性。方法 選取我院2014年1月～2016年1月收治并定期随访的糖尿病患者138例，按入组编号分为观察组和对照组，各69例，对照组给予常规糖尿病护理方式，观察组给予个案管理模式，比较两组患者的空腹血糖、糖化血红蛋白(HbA1c)水平、糖尿病知识、生活质量及自我行为管理。结果 干预前，两组患者FBG、HbA1c水平比较，差异均无统计学意义(P>0.05)；干预后两组患者FBG、HbA1c水平均有所降低，与干预前比较，差异有统计学意义(P<0.05)；观察组两指标评分均低于对照组，差异有统计学意义(P<0.05)。干预前，两组患者DKT、DSQL、自我行为管理(饮食控制、运动管理、足部护理、药物监测、高血糖处理)评分结果比较，差异均无统计学意义(P>0.05)；干预后观察组DKT、自我行为管理评分各项指标评分有所提高，DSQL评分有所降低，与干预前比较，差异有统计学意义(P<0.05)；对照组患者干预后各指标得分与干预前比较，差异无统计学意义(P>0.05)；观察组患者DKT、自我行为管理评分各项指标评分高于对照组，DSQL评分低于对照组，差异有统计学意义(P<0.05)。结论 在老年糖尿病患者护理过程中应用个案管理模式能够有效控制患者血糖，提高患者糖尿病自我管理能力，值得临床推广。

[Abstract]Objective To explore the feasibility of case management nursing model in elderly patients with diabetes mellitus.Methods 138 patients with diabetes mellitus in our hospital from January 2014 to January 2016 were selected and divided into the observation group and the control group，with 69 cases in each group.The control group was given routine diabetes care mode，while the observation group was given case management model.The levels of FBG，HbA1c，diabetes knowledge，quality of life and self behavior management were compared between two groups.Results Before intervention，there were no significantly different among the levels of FBG，HbA1c in two groups (P>0.05)；after intervention，the levels of FBG，HbA1c in two groups were all decreased，and compared with those before nursing，the differences were statistically significant (P<0.05).And those in the observation group was lower than those in the control group，and the differences were statistically significant (P<0.05).Before intervention，there were no significantly different among the scores of DKT，DSQL self behavior management (diet control，exercise management，foot care，drug monitoring，hyperglycemia processing) in two groups (P>0.05)；after intervention，the scores of DKT and all indexes in self behavior management in the observation group were increased，the score of DSQL in the observation group were decreased，and compared with those before nursing，the differences were statistically significant (P<0.05)，but there were no statistically significant differences in the control group between those before and after intervention (P>0.05).And the scores of DKT and all indexes in self behavior management in the observation group was higher than those in the control group，the scores of DSQL was lower than that in the control group，and the differences were statistically significant (P<0.05).Conclusion The application of case management model in elderly patients with diabetes mellitus can effectively control the blood glucose and improve the self behavior management ability of diabetes in patients.It is worthy to be popularized., 百拇医药(孔爱华)
