[摘要]目的 探讨4C延续性护理管理模式对脑卒中康复期患者的护理效果。方法 选取2015年9月～2017年3月我院收治的86例脑卒中患者作为研究对象，按照随机数字法将其分为观察组(n=43)与对照组(n=43)。对照组患者给予常规护理方案，观察组患者额外接受4C延续性护理管理模式。比较两组患者护理结束后的日常行为能力评分、遵医行为评分及护理满意度三个指标。结果 观察组患者出院第1、2、6个月的日常生活能力评分均明显高于对照组，差异有统计学意义(P<0.05)。护理后，观察组患者的遵医行为评分显著高于对照组，差异有统计学意义(P<0.05)。观察组患者护理满意度(90.1%)显著高于对照组(76.7%)，差异有统计学意义(P<0.05)。结论 4C延续性管理模式能大幅改善脑卒中患者的日常生活能力，提高护理的满意度，值得临床推广研究。
