重组人粒细胞巨噬细胞集落刺激因子凝胶治疗烧伤创面的临床效果观察(1)
[摘要] 目的 分析重组人粒细胞巨噬细胞集落刺激因子凝胶治疗烧伤创面的临床效果观察。 方法 选取我院从2014年6月～2016年6月收治的300例烧伤创面患者，随机分为两组，对照组给予创面涂抹安慰剂，实验组给予创面涂抹人粒细胞巨噬细胞集落刺激因子(rhGM-CSF)凝胶，比较两组患者全身状况、不良反应、创面愈合时间、愈合率以及总有效率等。 结果 实验组烧伤创面的平均愈合时间是(12.5±1.1)天，对照组烧伤创面的平均愈合时间是(16.9±1.2)天，实验组低于对照组，差异具有统计学意义(P<0.05)。在治疗第6、12 天后，实验组患者烧伤创面愈合率分别为53.12%、91.8%，对照组患者烧伤创面愈合率分别为36.11%、72.12%，实验组高于对照组，差异具有统计学意义(P<0.05)。在治疗第6、12天后，实验组第6、12天的总有效率分别为13.34%、91.68%，实验组第6、12天总有效率为1.67%、53.34%，实验组高于对照组，差异具有统计学意义(P<0.05)。 结论 给予患者进行rhGM-CSF凝胶治疗，愈合时间短，愈合率高等，安全有效，应用价值高，值得在临床上推广使用。

[Abstract] Objective To investigate the clinical effect of recombinant human granulocyte-macrophage colony-stimulating factor (G-CSF) in the treatment of burn wounds. Methods 300 patients with burn wounds treated in our hospital from June 2014 to June 2016 were randomly divided into two groups.The patients in the control group were treated with placebo，and the patients in the experimental group were treated with granulocyte macrophage (RhGM-CSF) gel.The systemic status，adverse reactions，wound healing time，healing rate and total effective rate of the two groups were compared. Results The mean healing time of the burn wound in the experimental group was (12.5±1.1) days.The mean healing time of the burn wound in the control group was (16.9±1.2) days.The experimental group was lower than the control group (P<0.05).The burn wound healing rate in the experimental group were (53±12)% and (91±8)%，respectively，and the wound healing rate in the control group were (36±11)%，(72±12)%， the difference between the experimental group and the control group was significant (P<0.05).The total effective rate of the experimental group were 13.34% and 91.68% on the 6th and 12th day after treatment，and the total effective rate were 1.67% and 53.34% on the 6th and 12th day in the experimental group，(P<0.05). Conclusion The rhGM-CSF gel treatment for the patients has the advantage of short healing time，highe healing rate，and high value.It is safe and effective.It is worthy of promotion in clinical use.

重組人粒细胞巨噬细胞集落刺激因子(rhGM-CSF)作用于造血祖细胞，促进其增殖和分化，其重要作用是刺激粒、单核巨噬细胞成熟，促进成熟细胞向外周血释放，并能促进巨噬细胞及噬酸性细胞的多种功能，从而可以促进创面的愈合[1]。我院自2014年6月～2016年6月收治的300例烧伤创面患者进行rhGM-CSF凝胶治疗。现报道如下。
1 资料与方法
1.1 一般资料
选取2014年3月～2016年6月我院收治的300例烧伤创面患者，实验组患者150例，男80例，女70例，年龄19～66岁，平均(32.5±2.7)岁，烧伤面积为(54±10)%，烧伤创面直径为(3.71±0.68)cm。对照组患者150例，男82例，女68例，年龄19～65岁，平均(32.4±2.8)岁，烧伤面积为(53±11)%，烧伤面直径为(3.64±0.73)cm。两组患者在年龄、性别、临床症状等一般资料比较，差异无统计学意义(P>0.05)。排除标准[2]：(1)创面是由电击受伤或者化学烧伤等原因引发的烧伤。(2)在4个月的时间内参加过其他药物临床实验。(3)处于哺乳期或者孕期。(4)对部分药物过敏或者对GM-CSF过敏。(5)患有心脏病、心绞痛、心功能不全、心肌梗死或者肝、肾疾病，精神疾病以及无自知力。(6)近期服用过免疫抑制剂。两组患者及其家属对本研究方案均详细了解，并自愿签署知情同意书，且研究方案已获得本院伦理委员会的批准。, http://www.100md.com(周伟明 叶运廷 李焕雄)
